Reversal & dosing notes

Sugammadex uses actual body weight: 2 mg/kg at reappearance of T2; 4 mg/kg at 1–2 post-tetanic counts with no TOF response; 16 mg/kg for immediate reversal approximately 3 minutes after rocuronium 1.2 mg/kg. These are different indications, not interchangeable doses. Not recommended in severe renal impairment (CrCl <30 mL/min).

Use quantitative neuromuscular monitoring. Rocuronium RSI here uses AdjBW, matching the original tool; protocols may use another scalar. Titrate anesthetics to response and account for age, comorbidities, and concurrent drugs.

6–8 mL/kg PBW is a ventilation reference, not a complete ventilator prescription; ARDS protocols generally start at 6 mL/kg PBW and require pressure-based adjustment.

Weights & formulas

03
TBW Total body weight
Measured weight in kg. 1 lb = 0.45359237 kg.
IBW Ideal body weight
Male: 50 + 2.3 × (height in inches − 60)
Female: 45.5 + 2.3 × (height in inches − 60)
PBW Predicted body weight
Male: 50 + 0.91 × (height in cm − 152.4)
Female: 45.5 + 0.91 × (height in cm − 152.4)
Used for tidal volume. The rounded inch and centimeter coefficients give slightly different results; IBW and PBW are calculated separately here.
LBW Lean body weight · Janmahasatian
Male: 9270 × TBW ÷ (6680 + 216 × BMI)
Female: 9270 × TBW ÷ (8780 + 244 × BMI)
AdjBW Adjusted body weight
IBW + 0.4 × (TBW − IBW)
BMI Body mass index
TBW ÷ height in meters². 1 inch = 2.54 cm.

TCI models have model-specific weight and BMI limits. Check the selected pump model and its instructions; do not substitute a calculated weight unless that model explicitly requires it.
